- Your full name, mailing address, and phone number.
- A statement consenting to the jurisdiction of the Federal District Court for your judicial district.
- A statement that you are making the submission in good faith and under penalty of perjury.
Upon receipt of a valid counter-notice, we may restore the removed content in accordance with DMCA procedures.

Toteaura maintains a policy of terminating, in appropriate circumstances, the accounts of users who are repeat infringers of intellectual property rights. Multiple DMCA notices against the same user may result in suspension or termination.
